Paulina Porizkova is taking care of her health.
The model, 58, revealed on Instagram Thursday that she is getting a "long overdue" double hip replacement surgery. Porizkova said her hips have been "worn down" and there is "none left" since being born with congenital hip dysplasia.
Her announcement included a photo of her in a two-piece bikini and another where she was seen lying down in a hospital room.
According to the Mayo Clinic, hip dysplasia can cause complications such as osteoarthritis or a hip labral tear. It occurs when "a hip socket that doesn't fully cover the ball portion of the upper thighbone."
"There is none left," wrote Porizkova. "The three doctors I've visited all recoiled when they saw my x-rays. 'Well, it's bone on bone,' they exclaimed. 'At least we know you're good with pain tolerance!' It's so bad, in fact, that doing one hip at the time is pointless. It would actually impede the healing and mobility."
Porizkova said she was happy to have had "one last vacation with old hips" and is ready for new ones.
"I'm so grateful this is a fixable problem," she continued. "I found a doctor I really trust, one who specializes in anterior hip replacements, I've had an amazing vacation … and now - like my sis-in-law said- 'take a break from kicking ass and let everyone wait on YOU for a change!'"
Although she admitted to being anxious about the procedure, she wrote that she was grateful to "have my Jeff [Greenstein] at my side, my mom and papa Joe, my boys and a slew of friends volunteering to come and bring me soup."
In an earlier Instagram post, Porizkova revealed that she and her partner had been spending time on an island in the Caribbean.
"As many of you who are my longtime IG friends may have noticed, I’ve been posting less than usual. That’s because I’m soaking up all the rays of happiness - understanding how precious and fleeting it all is. Simultaneously, I’m sparing you endless cutsie posts of 'look-at-what-fun-I’m-having' photos," she wrote in another Instagram post that had a selfie with Greenstein.

Living in the moment is something Porizkova told PEOPLE in October that she's been focusing more on doing.
"One of the incredible things about aging is that you care less and less about what other people think," she told PEOPLE. "You're like, 'You know what? I've done my bit. I've been serving everybody else for 50 years – now, it's my turn.'"
